Minimal invasive approachs for intrathoracic benign disease

Research Abstract
Benign lesions should preferably be approached through minimal invasive approach, although many surgeons still believe that minimal invasive surgery is not suitable in some benign intrathoracic pathologies due to size wise or adhesions from inflammatory process. We believe that every patient with a benign intrathoracic disease should be offered a minimal invasive approach as most of these patients are young, healthy with minimal or no symptoms so it is important to minimize chest trauma and its consequences. We would like to thank the authors of the letter to the editor “An incidental unique mediastinal mass in anasymptomatic young patient: thymolipoma “for their wonderful results but we just represent our case with nearly similar pathology, mediastinal lipoma, which was successfully managed by VATS approach.

Sewing needle penetration into the heart in an infant

Research Abstract
Sewing needle penetration into the heart is a rare occurrence. Clinical presentation and timing substantially affect the decision of surgeon and management approach. In this article, we report an 18-month case with a sewing needle penetration into the heart which was mistaken as foreign body aspiration showing migration. It should be kept in mind that sharp foreign objects can reach into the thoracic cavity through not only alimentary tract and airways, but also through externally penetrating trauma

Endoscopic ultrasound-guided choledochoduodenostomy for treatment of ampullary cancer

Research Abstract
We describe the case of a patient for whom choledochoduodenostomy was performed under endoscopic ultrasound (EUS) guidance as an alternative to percutaneous transhepatic biliary drainage (PTBD) for the treatment of obstructive jaundice. An 82-year-old man with ampullary cancer was considered operable, but he refused surgery. Endoscopic biliary drainage (EBD) with an 8.5- French plastic stent was performed 2 months later because of the development of obstructive jaundice. The EBD stent was occluded 5 months after the stent insertion, and EUS choledochoduodenostomy (EUS-CDS) was performed. Pneumoperitoneum occurred 1 day after the procedure, which resolved with conservative treatment. Six months later, multiple lymph node metastases occurred, and the patient was effectively treated by chemotherapy (S-1). The patient is still alive with a good quality of life more than 2 years after EUS-CDS. We conclude that EUS-CDS is an effective alternative to PTBD or EBD for patients with malignant biliary obstruction, especially due to ampullary cancer.

Localized isolated intrapericardial mesothelioma: a case report‏

Research Abstract
There are several cancers and diseases which affect the pericardial space around the heart, and pericardial mesothelioma is one of the rarest pathology. It is the most common primary malignant pericardial tumor. The clinical presentation of malignant pericardial mesothelioma includes chest pain, shortness of breath, coughing and severe nocturnal sweating. In this article, we report the fifth case of the localized isolated pericardial mesothelioma in the literature and the longest survival at two years reported so far
