Assessment of Nurses''''Knowledge and Practice Regarding Patients with Postextubation Dysphagia in Trauma Intensive Care Unit

Research Abstract

Nearly 60% of patients who are intubated in intensive care units (ICUs) experience dysphagia after extubation, and approximately 50% of them aspirate. Little is known about dysphagia recovery time after patients are discharged from the hospital. Aim of the study was to assessment of nurses'' knowledge and practice regarding patients with postextubation dysphagia in trauma intensive care unit.

Subjects and methods:

Descriptive design was utilized in this study on a convenient sample of 50 nurses''. The study was conducted in the trauma intensive care unit. Data was collected using a self-administered questionnaire with closed-ended questions which was developed by the researcher. The assessment was performed by using a questionnaire for knowledge and check list for practice assessment.

Results:

The study found that 40.0% of nurses'' were more 30 years. Regarding to educational qualifications, more than 50% of nurses'' had diploma nursing. The findings of study show that nurses have low overall knowledge and practice level.

In conclusion:

the overall knowledge of participants was found to be low knowledge about postextubation dysphagia. Regarding the overall practice of nurses'' was found to be low practice.

Recommendations:

Nurses in need for in-service training programs and refreshing courses to improve their knowledge, which will reflect into their practice while working with patients

Effect of Implemented Nursing Care on Breathing Discomfort and Weaning Outcomes for Patients Undergoing Mechanical Ventilation

Research Abstract

AbstractBackground: Breathing discomfort is one of the worst experienced symptoms by the mechanically ventilated patients in ICUs Aim: to evaluate the effect of implemented nursing care on breathing discomfort and weaning outcomes for patients undergoing mechanical ventilation. Design: A quasi-experimental research design. Setting: General intensive care unit at Assuit university hospital. Patients and methods: Convenient sample of 80 patients(40 patients for study group and 40 patient for control group) who were mechanically ventilated for >24 hrs. Breathing discomfort was evaluated according to patient conscious level and nursing care was done for study group and routine care for control group. Tools : Four tools were used for data collection in the study; Patient assessment, breathing discomfort evaluation: Part I :Respiratory distress observation scale(RDOS) and Part II: Dyspnea Visual Analogue Scale(D-VAS), Patient outcomes assessment and nursing intervention tool Results: High statistical significant difference between study group and control group in relation to arterial blood gases and haemodynamic parameters after intervention. Concerning weaning outcomes; 57% of control group experienced weaning failure versus 35% in study group Conclusion: Implementing nursing care among the mechanically ventilated patients were effective in improving patient outcomes. Recommendations: Apply nursing care for breathing discomfort as a routine care in ICUs.

The Effect of Enhanced Recoveries after Surgery Protocol on the Outcomes of Patients Undergoing Cardiac Surgery

Research Abstract

Background: Enhanced recovery after cardiac surgery protocol is an evidence-based interdisciplinary process, which has not previously been systematically applied to cardiac surgery. Objectives: The aim of this study was to evaluate the clinical effectiveness of ERAS protocol compared with routine care on the outcomes of patients undergoing cardiac surgery. Methods: This study was conducted between January 2020 and December 2020. A total of 75 patients who underwent cardiac surgery by one surgical team were evaluated for eligibility. Five patients were excluded after the initial assessment; hence, 70 patients were randomly assigned to the ERAS protocol group and control group. Patients in the ERAS group received all elements of the ERAS protocol while patients in control group receive routine care. Tools: Preoperative assessment tool to form base line data, Intra-operative assessment tool to assess Ischemic time, bypass time, and operation time and postoperative evaluation tool to assess the patients' outcome were used in data collection .Results: The duration of ICU stay and duration of mechanical ventilation were significantly shorter in the ERAS group versus control group3.04 ± 0.74, 2.33 ± 0.8), versus (5.82±0.61, 4.64±2.13), respectively; P < 0.001). Post-operative bleeding and re intubation were less in ERAS group versus control group (10 %, 3.33 %), versus (36.67 %, 16.66 %), respectively; P = 0.03).Conclusions: ERAS protocol reduces the length of ICU and for patients undergoing cardiac surgery.

Effect of stacked breathing exercise on reducing pulmonary infection and complications for patients with pleural effusion International

Research Abstract

Background: Chest expansion exercises are widely used for patients with pleural effusion. One of these exercises is stacked breathing exercise which has shown to be effective in mobilizing greater lung volumes and in achieving and sustaining deep inspiration. But it is not known whether it will have similar effects in patients with pleural effusion and if it has effect on reducing pulmonary infection and complications for these patients.

Objective: Was to evaluate the effectiveness of stacked breathing exercise on reducing pulmonary infection and complications for patients with pleural effusion.

Design: A randomized controlled experimental study was used in this study.

Participants: sixty patients with pleural effusion were selected by convenience sample and randomly assigned into two groups; the intervention and control groups (30 patients each).

Method: Three tools were used. Tool I was Pleural effusion assessment tool included demographic and medical history, hemodynamic parameters and assessment of respiratory system. Tool II is stacked breathing exercise tool used to assess the exercise done to the intervention group. Tool III was Patients' outcomes evaluation sheet used to assess the effect of stacked breathing exercise on pulmonary infection score and complications.

Results: There were significant differences in pulmonary infection score and complications between the intervention and control groups. Pulmonary infection score in the intervention group was much lower, and their complications were lower than those in the control group.

Conclusion: implementing stacked breathing exercise had significant effect in reducing pulmonary infection and complications in patients with pleural effusion than the routine hospital care.
