Impact of Nursing Intervention on Urostomy Patients Outcome Regarding Practicing Self-Care

Research Abstract
Urinary diversion operations (urostomy) patients reqUlre a great deal of emotional and physical adjustment. They usually face many problems such as loss of body function and relationship with others, and change in lifestyle. They need to adapt to the external appliance, stoma alteration in toileting habits, skin irritation problems, infection, and odor problems. Self-care for these patients is considered the most important factor that can minimize the complaints and complications. The aim of this study was to evaluate the impact of nursing intervention among urostomy patients regarding practicing self-care. The sample comprised 100 urostomy patients, divided into two groups: 50 study, and 50 control. For both groups, patients' knowledge about urostomy and related self-care performance were assessed before intervention, immediately after, and after six-months follow-up at outpatient clinic. Data were analyzed using Epi-info 6.04 computer software package. Findings revealed statistically significant improvements in knowledge and performance among patients in the study group, compared to the control group. The study group became more knowledgeable about the definition, function, anatomy, and physiology of urostomy, and in self-care performance. The rates of complaints and complications were found to be lower in the study group than in the control one. The study suggests that urostomy patients should receive their self-care training regarding urostomy during hospitalization. Follow-up at outpatient clinic is highly recommended as well.

HEALTH TEAM COMPLIANCE WITH UNIVERSAL PRECAUTIONS IN THE BURN DRESSING AND HYDROTHERAPY ROOM AT ASSIUT UNIVERSITY HOSPITAL

Research Abstract
Health team members continue to be at elevated risk of occupational exposure to a number of blood-borne and air-borne infectious diseases, compared to the general population. Universal precautions are protective behaviors designed to prevent the spread of blood-borne disease. The aim of this study was to measure health team compliance with universal precautions in _ the burn department in Assiut University Hospital, and to identify the barriers that might underlie any noncompliance. Across-sectional analytic research design was utilized in this study the tools were utilized for data collection, interview questionnaire form and observation check list. The sample of this study consisted' of all health team members working in the burn department (dressing and hydrotherapy room) at Assiut University Hospital. They were 18 physicians, 46 nurses, and 1-6 housekeepers. More than half of the nurses and physicians, 52.2%, and 66.7% respectively, were in the age group 20 to less than 30 years, while the majority of housekeepers were 4(} years old or more (75.0%). Almost all nurses (95.7%) had a diploma degree, and only two (40.3%) had a bachelor degree in nursing and the remaining had a master degree. The majority (88.9%) had less than jive years of experience. Most Housekeepers (87.5%) had ten or more years of experience. Health team members in the buns department had deficient knowledge and poor compliance to Universal precaution, especially the Housekeepers. this study concluded and recommended that conduct specialized in-service training programs in universal precautions. These should be in the form of interactive programs compatible with adult learning principles, and in which health team members practice role-play to demonstrate how they could protect themselves from hazardous occupational exposure. Also, supplies should be provided insufficient quantities, but taking into consideration of the limited resources of the country. Further studies are suggested to investigate the psychosocial and organizational factors that correlate with deficient practices, and to assess the relationship between wOl"k organization factors and workplace injuries and occupational illness.

Evaluation of Parenteral Nutrition on The Immune System of Patients with Poly trauma Mervat Anwar Abd El-Aziz Assistant Lecturer

Research Abstract
Nutritional support for trauma and critically ill patients has been advocated to promote immune system recovery and to prevent as much tissue break down and nutritional deficit. The present study aiming to assessment of the parenteral nutrition on the immune system of patient with multiple trauma. Compare the effect of early parenteral nutritional and conventional hospital diet 0" the nutrition and ·immunology markers in trauma patients. A convenient sample of 40 severely traumatized patients within 8 hours after trauma they were enrolled sequentially into conventional (n = 20) and total parenteral nutrition (n= 20) groups (TPN). Criteria for subject inclusion were, age 18· 45 years, Injury severity score (ISS) 20-40 score. Tools of data collection, four tools were utilized to collect data pertinent to the study. These tools were, nutritional assessment sheet, trauma scales and (APACHE) score (checklist), parenteral nutrition plans, evaluation tool which includes (Anthropometric measurement, Laboratory studies Immunological parameters). Total parenteral nutritional has positive impact for trauma patients on anthropometric measurement, (weight loss, TSF and MAC) which was observed at the end of therapy (7th day) for the conventional group while there were no changes for the same parameters in TPN group. As regard, laboratory tests, there were significantly increase in the values of total serum protein and albumin among patients of the TPN group after the end of therapy at (7th day) .As regard immune function, it was found that the mean values of (IL-lO, lL-6 , CD8) were significantly dropped in the TPN group after the end 0'£ therapy as compared to the admission. Also there were significantly increase in the values of complement C3, lOA, CD4, and CD4/CD8 ratio among patient of the TPN group at (7th day). Establishment and implement of standardized protocol of parenteral nutrition support using nutritional screening and physical assessment forms, for the critically ill patients.

Relations Between Obesity, Triglycerides and Blood Glucose Levels Among University Employees in Lower and Upper Egypt

Obesity has reached epidemic proportions globally, with associated increased risks of coronary heart disease, hypertension and type 2 diabetes mellitus. It is essential that nurses be knowledgeable regarding obesity prevention and health promotion. The aim of this study was to investigate the relation between obesity and triglycerides and blood glucose levels among University employees in Egypt and to compare these parameters in Lower and Upper Egypt. The study was conducted at different faculties at Lower (Banha, EI-Mansoura, Zagazig) and Upper (Assiut) Egypt universities. Comparative Study Between two Nutritional Regimens on Wound Healing of Moderate Burned Patients

Research Abstract
Bums are manifestations of injuries that are typically inflicted on the skin and other epithelial tissues. The metabolic disturbance and fluid imbalance are direct results of burn trauma and of fluid losses from the bum wound. Hypermetabolism and hypercatabolism characterize the body's response to a burn injury resulting in increase in energy and protein requirements. This study aimed to determine the effect of two nutritional regimens on wound healing of moderate burned patients. It was carried out at the burns unit of the Assiut university hospital, from October 2002 till December 2004. A sample included lOO adult recent moderately burned patients assigned sequentially into a group I (n=50) who their calculated caloric requirement form natural nutrients including the ordinary hospital diet, and group II (n=50) who received outline hospital diet & supplementation by commercial products rich in protein and carbohydrates (sustagen or Ensure). Monitoring the effect of &0 regimens was done by laboratory investigation. and anthropometric measurements. The main results reveal significant increase in weight gain in group Il compared to group I at discharge. There was an increase in value of mid-arm muscle circumference (MAMC) body mass index (BMI) and triceps skin fold thickness(TSF) on discharge than 011 admission. In addition, a significant relation ship of serum transferrine, albumin and serum protein among patients groups.

Factors affecting communication between second year nursing students
and patients at Assiut University Hospital

Research Abstract
Communication is a key tool that health care professionals must use to the elicit cooperation of many individuals in the delivery of health care services, communication is essential for effective nursing practice and establish the type of relation ship with a patient that influences the out come of care. The present study aimed to investigate the factors affecting communication between patients and second year faculty nursing student. The study was conducted at Assiut University hospital as clinical areas for training of second year nursing students ( orthopedic, urology neurology and burn units). Convenience sample from the second year nursing students of the academic year 2006/2007 ( one group n=45) and 96 patients who communication with the secand year nursing students at the areas have been selected. Data were collected through patients opinionair sheet to assess patients' opinion about nursing students communication, students interview questionnaire sheet to assess the nursing students' knowledge and practice in relation to communication. The data collected during October and November 2006/2007. Results of this study, revealed that most of the nursing students had satisfactory knowledge about communication and most of them had adequate communication skills. 82% of them, their age was 18 years old and more than half of them were from rural areas patients opinionair about communication towards second year students in relation to their socio demographic characteristic, it observed that the statistically was significant regarding to marital status, age, females, uneducated, unemployed and orthopedic department. The study concluded that, the factors affecting communication were age of the students and patients, sex of patients, level of knowledge, amount of training, and students skills while receiving patients, dealing with patients, talking and listening to patient and environment around them. The study recommend ensuring appropriate clinical supervision of student and patient. Implementation of teaching programs to the student nurses about communication are highly recommended.

Relationship between Assertiveness and Leadership Styles among Nurses' Managers at Assiut University Hospital

Research Abstract
The aim of the study was to assess the relationship between the assertiveness and leadership styles among nurses' managers at Assiut University Hospitals. The study sample including all the available nurses' managers (82) currently assigned to work in different units at Assiut University Hospitals; (1) director of nursing, (3) Assistant director, (12)supervisors, and (66) head nurses. The questiorUlaire sheet consisted of three parts: The first part related to sociodemographic characteristics of the study sample (age, qualifications, position, and years of experience). The second part of the tool consisted of 34 questions that measure the assertiveness of the nurse managers. The possible responses of this part were strongly agree scored (I), agree scored (2), sometimes scored (3), disagree scored (4) , and strongly disagree scored (5). The third part of the tool consisted of 16 questions that measure the leadership style of the nurses' managers. The possible responses of this part were yes scored (I), and no scored (0). The questionnaire is divided into three parts (each part involves six questions) the first six questions detected the directive leadership style, the second six questions detected the participative leadership style, and the last six questions detected the permissiveness leadership style. The result denoted that there were no statistically significant differences between leadership styles and assertiveness among the nurses' managers. On the light of the present findings the researchers recommended the necessity of sharing of the nurses in decision - making, policy making, informing the nurse managers with description of each position, the nurses' managers must have authority commensurate with their responsibilities, and must have adequate opportunity for promotion, professional growth and inservice training opportunity about communication and leadership in nursing.
