Impact of Postoperative Rehabilitation Program on Outcomes of Patients Following Cervical Discectomy

Research Abstract

Background: Cervical disc herniation is the most common cause of neck and arm pain in adults. Postoperative rehabilitation is an essential part of the recovery period. Aim: The aim of the study was to evaluate the impact of postoperative rehabilitation program on outcomes of patients following cervical discectomy. Subjects and Methods: Research design: Quasi-experimental design was used. Sample and Setting: A random sample of 70 adult and old adult patients from both sex was recruited from July 2021 to July 2022, in addition to 6 month follow-up period that ended in January 2023 in the neurosurgery department and neurosurgery outpatient clinic at Assiut University Neurological, Psychiatric, and Neurosurgery Hospital. Patients were assigned randomly by a computer-based selection program into 2 equal groups (control group n=35) and (study group n=35) with a ratio 1:1 assignment. A control group received routine hospital care/instructions while a study group received routine hospital care/instructions in addition to the structured postoperative rehabilitation program by researchers. Tools: assessment sheet for patients undergoing cervical discectomy, numeric pain rating scale, neck disability index, and Newcastle satisfaction with nursing care scale. Results: Study findings revealed a statistically significant improvement for both groups, however, improvement was higher in the study group than in the control group (p. value < 0.01) post application of the postoperative rehabilitation program as regards all outcome measures; pain, disability, and satisfaction. Effect of Combined Cognitive and Motor Intervention on Muscles Strength, Balance and Daily Activities among Elderly at Risk for Falls

Research Abstract
Background: One in three seniors falls at least once a year, making falls a serious public health concern. Increased mortality, injuries, loss of independence, and unfavorable psychosocial effects are all linked to falls. Aim of the study: To evaluate the effect of combined cognitive and motor interventions on muscle strength, balance, and daily activities among elderly people at risk for falls. Subjects and method: Design: A quasi-experimental study (study/control) was used in the current study. Setting: the study conducted at six geriatric homes at Minia governorate. Subject: total coverage of the elderly at selected places was comprised in the study. Tools of data collection: Four tools were used to gather data for this study: Structured interviewing sheet; Medical Research Council Scale for Muscle Strength and Berg Balance Scale and The Katz Activities of Daily Living. Results: It was found that, combined cognitive & motor exercises group had the highest scores for muscle strength, maintain balance, and becoming independently in performing activity of daily living (ADLs) post 24-week of exercises, compared with the control group. Conclusion: Combined cognitive and motor exercise interventions had an adequate influence on the improvement of balance, gait, activity of daily living, and muscle strength in the upper and lower limbs and decreased the risk of falling compared to motor intervention alone. Recommendation: Health education among elderly about the effects of combined and cognitive training exercises on balance, muscle strength, and activities of daily living through effective teaching media such as videos, role play, and demonstration.

Effectiveness of Educational Nutritional Sessions on Quality of Life, Functional and Nutritional Status among Elderly Patients with Chronic Kidney Diseases

Research Abstract

Background: Elderly patients affected by chronic kidney disease (CKD) and end-stage renal disease (ESRD) are rapidly increasing. Elderly renal patients are frail and more susceptible to impair the nutritional status. Nutrition in elderly patients with chronic kidney diseases is important in decreasing complications and improving their quality of life (QOL). Study aim: To detect the effect of educational nutritional sessions on quality of life, functional and nutritional status among elderly patients with CKD. Design: Quasi-experimental research design. Settings: This was an interventional study that was conducted in the hemodialysis unit at Assiut university hospital. The program was conducted on elderly participants with CKD. Tools: Five tools were used to conduct the study. Tool I: Structured interview questionnaire: It include socio-demographic characteristics and medical data. Tool II: Kidney Disease Quality of Life Short Form Arabic version (KDQOL-SF). Tool III: Karnofsky Performance Scale Index. Tool IV: Mini Nutritional Assessment Scale. Results: the present result revealed that the elderly patients’ QOL had improved as high quality of life score increase to more than two fifth in posttest & follow up for study group. Also there was statistically significant difference between pretest and posttest regarding functional and nutritional status P= 0.000*. Conclusion: The results indicate that the nutritional educational sessions had significant positive effect on elderly patients’ QOL, nutritional, and functional status before and after intervention. Recommendations: Conduct Periodical educational and training program to elderly and their caregivers about therapeutic nutrition to improve their quality of life, functional and nutritional status.

Effect of Blinking Exercises on Eye Dryness and Quality of Life for ElderlyPeople

Research Abstract

Background: Eye dryness is a common health problem among older adults which negatively impacts their quality of life. Aim: To assess the effect of blinking exercises on eye dryness and quality of life for elderly people. Design: A quasi-experimental research design (Pre/post-test) was utilized. Sample: A purposive sample of one hundred elderly patients who complained from dry eye was selected. Setting: The study was conducted in ophthalmology outpatient clinics at Minia University Hospital, Egypt. Methods: Three validated tools were used 1) A structured interview questionnaire sheet involving personal and medical profiles. 2) Ocular Surface Disease Index (OSDI) questionnaire, and 3) Impact Dry Eye Everyday Life Questionnaire. Results: The mean age of the studied sample was 68.62±8.62, and 59.0% were female. It was found that the severity of eye dryness declined after the implementation of eye blinking exercises to 5% versus 15% before the intervention at a mild level and declined to 15% at a moderate level compared with 40% before with a high statistical significance difference P value < 0.00001*. Additionally, the effect of eye dryness on quality of life diminished with a statistically significant difference between pretest & after 4 weeks of the intervention reflected by P-value=0. 0352. Conclusion: Applying eye blinking exercises effectively reduced eye dryness and improved the quality of life among older adults. Recommendation: Health education and training should be provided regularly in eye blinking exercises in different healthcare settings for elders with the availability of related posters, brochures, and handouts given to them.
