The effect of patients positioning on oxygen saturation in the acute phase of ischemic stroke

Research Abstract
Body position is known to influence respiratory function in normal subjects & those with respiratory pathology. Its effect on respiratory function after stroke has received little attention. It's generally thought that stroke morbidity is increased by appropriate decision early recovery phase. So the present study was designed to identify changes in arterial oxygen saturation (Sa02) associated with different four positions of the patient in order to detect proper position which maintain the optimal level of Sa02. Quasiexperiment research design was applied in this study. This study was conducted in stroke intensive care unit of neurological department of Assiut University Hospitals. The subjects of this study consist of 60 patients with acute ischemic stroke within 48 hours following mild to moderate and severe stroke. Three tools were included in the study, tool of sociodemographic data, tool of the positions, & tool of the Scandinavian stroke scale. The result of the present study revealed that there was statistical significant difference between Sa02 & blood pressure of the patient before & after the positions. It was found also that total mean of Sa02 during right side, supine & semi sitting positions was nearly equal and that there was statistical significant difference between left position and 3 other positions. So the study recommended the importance of changing patient's positions which improve ventilation & blood pressure. The importance of the use of positions tested CRt. Side, supine & semi sitting positions) in the clinical practice to maintain Sa02 in patients with ischemic stroke.

Impact of counseling program for adolescent and adult epileptic
patients and their families in Assiut University Hospital

Epilepsy is a manifestation of brain neural hyper excitability. It is a symptom complex eNS disorder of a variety underlying etiologic causes. Seizures occun'ing across the spectrum of age, gender, race, and economic background. The aim of the study was conducted to investigate the impact of counseling program for adolescent and adult epileptic patients and their family also implement and evaluate the out come of a counseling program for the families and epileptic adolescent and adult. The study was carried out in epileptic out patient clinic of Assiut University Hospital. The design is descriptive design (pre, post, and follow up after six months). The total of epileptic adolescent and adult 100 and 50 families of epileptic patients. Two tools were used to collect data 1- Questionnaire sheet to assess family and epileptic adolescent and adult knowledge and care for epileptic. 2- Structured observations check list to assess family performance during se izures. 46% of male and female epileptic adolescent and 43% ranged from 18-40 years of male and female epileptic adult. Overall 20% ranged from 40-50 years. The occupation and education were associated significantly with epileptic adolescent and adult knowledge. This difference was statistical significant. On the other hand, there was significant decrease in the trend of total prevalence of epilepsy due to environmental, hereditary and trauma. The study recommended: Establish a counseling health team to guide the family of the epileptic adolescent and adult health education to effect significant on patients knowledge, performance related to care of epilepsy and recommended to conduct an applied training program on care of epileptic adolescent and adult to the nurses to educate them for every epileptic patients.

Decentralization and its impact on nnrses' autonomy, job satisfaction, and organizational commitment at Assuit University Hospitals

Research Abstract
This research lunched to determine to what extent decentralization influences nurses autonomy, job satisfaction, and organizational commitment. There are studies which show that decentralization decision making increases productivity, improves moral, increases favorable attitudes, and decrease absenteeism. Decentralization of decision making is often facilitated by participative management strategies whereby individuals thought the organization are given more information, knowledge, and power. This research was conducted in inpatients units Assuit University Hospitals and number of head nurses participated in the study. Data were collected through a questionnaire form including three different tools. The results of the present study revealed that the mean score of head nurses organizational commitment and job satisfaction was and low as regarding to decentralization. There was a direct effect of decentralization on organizational commitment. Decentralization was the only significant predictor of autonomy. Managers should diagnose the organizational situation and select the decision making level that will best meet the organization needs. When determining the optimal degree of decentralization organization, top management needs positive attitudes toward decentralization, they need competent personnel to whom they delegate authority and access information necessary for decision making.
