Effect of Tele-nursing Instructions on Adherence to Therapeutic Regimen and Improving Symptoms for Patients with Peptic Ulcer

Research Abstract

Back ground:Peptic ulcer is an excavation that forms in the mucosal wall of the stomach,
pylorus, duodenum and in the esophagus. Tele-nursing has a great contribution to ensuring
continuity of care for patients following hospitalization. Aim: To evaluate effect of tele-nursing
instructions on adherence to therapeutic regimen and improving symptoms for patients with peptic
ulcer.Patients and Methods: Research design: Aquasi experimental research design (study and
control). Setting: Tropical Medicine and Gastroenterology department at El - Rajhi liver Hospital at
Assuit University hospital .Sample:A purposive sample of 60 male and female patients diagnosed
with peptic ulcer.Tools: Tool (I):Patient's assessment sheet.Tool II: Morisky Medication
Adherence Scale (MMAS-8).Results: Statistically significant difference in adherence level of study
group before and after telenursing instructions (2.03±1.67&6.93±0.91) respectively. While in
control group there was no statistically significant difference in adherence level (2.13±1.38&
3.6±1.43).Also , there was improving in symptoms for patients with peptic ulcer among study group
than control group after applying tele-nursing instructions. Conclusion:Telenursing instructions
improved adherence level of patients to therapeutic regimen and improved symptoms for patients
with peptic ulcer Recommendations: Further studies aimed at improving patient adherence using
telenursing strategiesf or patients with peptic ulcer should be applied.

Effect of nursing guidelines on first‑year nursing students’ knowledge and reported practice towards sensory impairments among the elderly

Research Abstract

Abstract
Background: Sensory impairments are common among older adults. These impairments have consequences on
activities of daily living and communication with others. Such impairments for the elderly remain a significant public
health issue globally. This study aimed to assess effect of nursing guidelines on first-year nursing students’ knowledge
and reported practice towards sensory impairment among the elderly.
Method: A Pre- and post-test research design was utilized in this study to assess first year nursing students’ knowledge
and reported practice towards sensory impairments among the elderly. It was carried out at faculties of nursing
affiliated with three universities with a purposive sample (n = 531) of the first-year nursing students. The study was
conducted in four phases: Pre-intervention assessment, nursing guidelines development, nursing guidelines implementation,
and post-test after one month. The pre- and post-tests were conducted online and included three parts
to collect the required data about students’ socio-demographic data, students’ knowledge about the five senses and
changes in these senses among the elderly, and the students’ reported practice for coping with changes in these
senses among elderly. Student t-tests and an ANOVA test were used to compare means. For qualitative data, comparison
was done using chi-square. Pearson correlation coefficient was used for detecting the relations between continuous
variables of the study.
Results: There are statistically significant differences between the studied subjects means score knowledge and
reported practice about the five senses among elderly people in the pre- and the post-tests (P = 0.001). At pre-test
the total score mean of students’ knowledge was 24.25 while at post-test became 28.16. At pre-test the total score
mean of students’ reported practice was 38.40 while at post-test became 44.43. There is a relationship between students’
knowledge and their reported practice at both pre-test and post-test with P value = 0.001.
Conclusion: The levels of the first-year nursing students’ knowledge and reported practice of the studied sample
towards sensory impairment among the elderly were improved after implementation of the nursing guidelines. So,
it is recommended that these nursing guidelines could be embedded within the undergraduate curriculum. Raising
students’ awareness through providing lectures, and workshops on sensory impairment among elderly and how to
deal with them, and train students on how to communicate with sensory impairment among the elderly.

Effect of Swallowing Exercises on Swallowing Function for Patients with Neurogenic Dysphagia

Research Abstract

Background: Neurogenic dysphagia is one of the most common dangerous disorders for patients with neurological diseases. Aim: The aim of the study was to evaluate the effect of swallowing exercises on swallowing function for patients with neurogenic dysphagia. Patients and Methods: Research design: Quasi experimental (pre/post test) research design was used. Sample and Setting: A selected sample of 60 patients diagnosed with neurogenic dysphagia was recruited and also followed up for 2 months at neurology department and neurology outpatients clinic at neurology, psychiatry and neurosurgery Assiut University Hospital. Each patient was oriented and trained by the researchers to know the effect of practicing swallowing exercises and received brochure to be oriented about its effect on swallowing function. Tools: Patients assessment questionnaire, functional oral intake scale, Bazaz dysphagia score, eating assessment tool-10 and the 8-point penetration-aspiration scale. Results: The study finding revealed a highly statistical significant improvement (p < 0.0001) post application of the swallowing exercises as regard all outcome measures; functional oral intake scale, Bazaz dysphagia scale, aspiration and complications. Conclusion: Swallowing exercises significantly improve swallowing function and decrease severity of neurogenic dysphagia, aspiration and complications. Recommendation: The swallowing exercises (brochure) should be utilized in neurology hospitals as a teaching guide for patients with neurogenic dysphagia.

Background: Diabetic patients with poor health behaviors are at greater risk for a range of adverse health problem. Diabetes foot problems can be avoided by practicing self-care. Study aim: evaluate the impact of health intervention about foot self-care behavior among adult and elderly diabetic patients. Subjects and Methods: Quasiexperimental design pre and late posttest was applied in Internal and the outpatient clinic at main Assiut university hospital. The sample size: This study included purposeful sampling, with a total of 80 diabetic participant. Data collection tools: Interviewing questionnaire included personal characteristics, medical data, Diabetic Foot Self-care behavior scale and Health Intervention through International Working Group Diabetic Foot. Results: Mean ±SD of Diabetes Mellitus duration was 10.7 ±8.2 years, significance difference between the total score of foot self-care behavior in pre and late posttest with a mean ± SD (42.8 ±6.96 and74.04±29.11) respectively. Also, between selfcare behavior of the studied sample and their knowledge level in pre & late posttest (P. value= 0.061 & 0.001) respectively Conclusion: Health intervention have a positive significant impact on the studied sample self- care behavior regarding diabetic foot. Recommendations: Provision of comprehensive foot care and intervention for diabetic patients with foot ulcers in order to avoid complications.

Research Abstract

Aging is an inevitable part of life and the largest risk factor for cardiovascular disease (CVD). Aim: Assess the effect of health promotion nursing intervention on the functional status among elderly with cardiovascular diseases. Design: A Quasi-experimental design was used. Settings: The study conducted in cardiac out patients' clinics and inpatients wards at Al-Orman hospital in Assiut university hospitals. Sample: A random sample of 70 elderly with CVD was divided equally into two groups (35 study and 35 control). The data collection lasted 7 months from the beginning of December 2020 and ending in late June 2021. Tools: Four tools were utilized. Tool I: Structured interview questionnaire, Tool II: Comprehensive Heart Disease Knowledge Questionnaire, Tool III: Instrumental Activities of Daily Living Scale (IADL), Tool IV: Katz Index for Activities of Daily Living. Results: It was noted that 80.0% of the study group achieved full functional level in posttest. Additionally; a statistically significant difference between study group function level and their knowledge level at P<0.001. There was positive correlation between ADL, IADL and cardiac diagnosis in study group y-0.989. Conclusion: Functional level of elderly patients with cardiovascular diseases was greatly affected by a health promotion nursing intervention. Recommendation: Providing further health promotion nurse intervention for elderly with cardiovascular diseases in all health care facilities
