Prevalence of Alzheimer's disease and other dementing disorders: Assiut-Upper Egypt study.

Research Abstract
Abstract The aim of this study was to determine the prevalence rate of Alzheimer's disease (AD) and other types of dementias among the population over the age of 60 years in Assiut governorate. A 3-phase cross-sectional population-based study was carried out to screen 2, 000 subjects residing in 11 different locations representing the sociocultural status of the area. We subjected each proband to a modified form of the Mini Mental State Examination test. Subjects scored 21 or less were investigated according to a standardized protocol. We found 90 demented subjects yielding a crude prevalence ratio (case per 100 population over the age of 60) of 4.5. A diagnosis of subtypes of dementia was reached in 83 cases. Prevalence ratios for dementia subtypes were 2.2 for AD, 0.95 for multi-infarct dementia. 0.55 for mixed dementias and 0.45 for secondary dementias. Age-specific prevalence tends to be doubled every 5 years. Occupation, level of education and residence did not affect the prevalence or severity of dementia. Comparison with other studies suggests that dementia of all types is as frequent in Assiut governorate as elsewhere.

N-acytelcysteine prior to intrauterine insemination in couples with isolated athenospermia: a randomized controlled trial

Research Abstract
Objective: to compare semen parameter and pregnancy rate after treatment of couples with isolated mild athenospermia with N-acetylcysteine (NAC) or not prior to intrauterine insemination (IUI). Design: open-labeled randomized controlled trial Material and Methods: All couples presented with primary or secondary infertility to the outpatient infertility clinic a university affiliated hospital and welling to perform IUI as a part of their infertility management were approached for participation. We had included only couples with mild athenopermia (progressive motility between 10-32%) with normal other seminal criteria and normal infertility workup for a female partner. Couples were randomized to either one of two groups: NAC group (30 couples) who received 600 mg NAC for 12 weeks prior to IUI and a control group (30 patients) who did received no other medication. The results as regard the seminal quality after seminal preparation as well as chemical and clinical pregnancy rate were recorded. Results: Both treatment groups were homogenous at the time of randomization regarding the type and duration of infertility. The mean sperm count and percentage motile were homogenous at the time of recruitment. NAC group produced a significantly higher retrieved mean sperm concentration compared with the control group 36.62±9.2 versus 31.9±10.61 million per high power filed (p=0.041), respectively. The percentage of progressive sperm motility (grade A) was significantly higher in the NAC group 22.53± 11 compared with the control 18.7±7.81 ( p= 0.032). Clinical pregnancy encountered in 6/30(20.0%) of NAC group compared with 4/30(13.3%) of the control group (p=0.25). Conclusion: N-acetyl-cysteine treatment for two months prior to IUI in couples with isolated athenospermia improvers the seminal quality after preparation. It also improves the chemical and clinical pregnancy rate.

Peripheral neuropathy in burn patients.

Research Abstract
Abstract Neuropathy in burn patients is frequently overlooked. This study aimed at looking for neuropathies among burn patients. It included 55 burn patients, whether symptomatic or asymptomatic, with variable depths of burn at different stages. Their ages ranged from 8 to 55 years with a mean age of 23.6 +/- 11.1 years. All patients were submitted to clinical examination, electromyographic and motor conduction velocities of burned and unburned limbs. Serum electrolyte, blood urea and creatinine were measured for all patients. Sixteen patients (29 per cent) had peripheral neuropathy. Only six had symptoms and signs of peripheral neuropathy. The most frequently diagnosed neuropathy in this study was mononeuritis multiplex in nine patients (56 per cent), then generalized distal axonal neuropathy in five patients (31 per cent) and entrapment neuropathy in two patients (13 per cent). In patients with mononeuritis, 29 nerves were affected, 24 nerves related to the site of the burn and five nerves were away from the site of the burn. All the entrapment neuropathy developed after wound healing. Age above 20 years, electric burns burns involving full thickness of the skin and a surface area of more than 20 per cent were associated with a significantly higher prevalence of neuropathy. Other parameters were not found to be significant in the development of neuropathy. PMID: 9568328 [PubMed - indexed for MEDLINE]
