Incidence and clinical predictors of outcome of Bell's palsy, Al-Quseir City, Red Sea Governorate, Egypt

Research Abstract
Background Bell's palsy (BP) is one of the most common causes of acute-onset unilateral facial weakness. Through this study we aimed to estimate the incidence of BP in Al-Quseir City, Red Sea Governorate, Egypt. Patients and methods A project was undertaken to assess the epidemiology of major neurological disorders. A total of 33 285 eligible patients were screened through a door-to-door survey (every door) by three specialists in neurology and 15 social workers. All patients were subjected to detailed history taking and a meticulous neurological examination by means of a specific questionnaire designed for this study. Results Within 1 year, 27 patients were diagnosed with BP. This yielded an incidence rate of 98.9/100 000 population (aged 9 years and older). The incidence was higher in the male population than in the female population (116.4 and 81.2/100 000, respectively). Age-specific incidence of BP showed that its …

One step diagnosis of multiple sclerosis disease activity, dissemination in time and space using diffusion weighted MRI

Research Abstract
Objectives To assess the value of DWI with its two components (DWI-b1000 and DWI b0) images in assessment of active MS and in defining dissemination in time (DIT) and dissemination in space (DIS) in those patients. Methods Brain MRI studies were performed using T1WI, T2WI, FLAIR and DWI and CE T1WI. Results Twenty-one patients (Males/Females= 6/15) of age range 18–50 years were included. Forty lesions were hyperintense on DWI-b1000 that had significant link to enhancement seen on contrast- enhanced T1WI (100%) indicating disease activity. Using the 2010 revised McDonald criteria: In 9 patients (42.85%), there were [Asymptomatic active lesions (n= 17)]. Those lesions were simultaneously present along with [Non-active demyelinating lesions]; DIT could be diagnosed in these cases based alone on MRI findings. In 12 patients (57.14%) …

Prognostic role of neurophysiological testing 3–7 days after onset of acute unilateral Bell's palsy

Research Abstract
Summary Objective Recovery from acute Bell's palsy (BP) is variable and there are few predictors of response. We evaluated the usefulness of a range of neurophysiological parameters to predict outcome in BP. Methods Fifty-nine patients (age: 33.7±15.4 years) with acute unilateral BP were recruited within 3–7 days of onset. They were evaluated with electroneurography, facial nerve excitability, and the blink reflex. House-Brackmann (HB) clinical scores were obtained at the same time and three months later. All patients received prednisolone treatment and regular rehabilitation. Results At three months, 41 patients (69.5%) had good recovery, while 18 patients (30.5%) had poor recovery according to the HB scale. The facial nerve excitability threshold and threshold difference between sides were significantly lower in patients with good recovery than those with poor recovery (P …
