Twelve month outcome of manual small-incision cataract surgery in Assiut, Egypt: A retrospective study with a large sample size

Research Abstract

BACKGROUND:

To assess the visual outcome of manual small-incision cataract surgery (MSICS) as well as safety, cost, and time of the procedure.

PATIENTS AND METHODS:

A retrospective study involving candidates for cataract surgery with baseline-corrected distance visual acuity (CDVA) ≤20/120. Visual acuity (VA) was the primary outcome measure while surgical complications, cost, and time of surgery were the secondary outcome measures. Follow-up visits were scheduled at 1 day, 1 week, 1 month, and at 6 and 12 months following surgery.

RESULTS:

The study enrolled 3007 patients with a mean age of 66.45 ± 17.3 years. Out of 3007 patients, 2774 (92.2%) were legally blind before surgery (CDVA <20/200) which was significantly reduced to 55 patients (1.9%) by 1 month following surgery. Uncorrected distance visual acuity was 20/60 or better in 2098 eyes (69.8%) at 1 month, in 2035 eyes (67.7%) at 6 months, and in 2017 eyes (67.1%) at 12 months. The posterior capsular rupture was the most common intraoperative complication. Corneal edema was the most common immediate postoperative complication while the development of posterior capsular opacification was the leading cause for later impaired VA. The mean cost was approximately equivalent to 20 US dollars. The median duration of surgery was 10 min.

CONCLUSION:

MSICS is a safe, cost-effective, and time-saving technique for improving the vision of cataract patients in areas with high cataract surgery volume and limited facilities.

Preservative-Free Topical Anesthetic Unit-Dose Eye Drops for the Management of Postoperative Pain Following Photorefractive Keratectomy

Research Abstract

Introduction

Ocular pain is a common complication following photorefractive keratectomy (PRK). The level of patient satisfaction with current pain control strategies is not high. This study aims to assess the efficacy and safety of a novel regimen of preservative-free oxybuprocaine hydrochloride 0.4% unit-dose eye drops for post-PRK pain control.

Methods

In a contralateral eye study, 144 eyes of 72 patients who underwent bilateral transepithelial PRK (TransPRK) were stratified into experimental and control groups. The experimental group received preservative-free oxybuprocaine hydrochloride 0.4% unit-dose eye drops five times daily postoperatively until complete epithelial healing, while the control group received sodium hyaluronate 0.2% instead. The main outcome measures were pain scores assessed by the verbal rating scale and visual analogue scale (VRS, VAS), the corneal epithelial defect (CED) area, epithelial healing duration evaluated by slit-lamp biomicroscopy and anterior segment optical coherence tomography (AS-OCT), and endothelial cell density (ECD) measured before and 1 month after surgery.

Results

Pain scores assessed by VRS and VAS were significantly lower in the experimental group 8 h after surgery, and 1, 2, and 3 days postoperatively (P < 0.001). The mean CED area showed no significant differences between the two groups at different follow-ups (P value > 0.05). The corneal epithelial healing had a mean duration of 3.32 ± 0.47 days in both studied groups and was parallel in both eyes of each patient. In each group, 49 eyes (68%) and 72 eyes (100%) had a fully epithelialized surface on the third and fourth postoperative days, respectively. No significant changes were observed in the mean ECD 1 month following surgery in both groups (P value > 0.05).

Conclusion

Preservative-free oxybuprocaine hydrochloride 0.4% unit-dose eye drops are effective and safe in controlling early postoperative pain following TransPRK. The availability of the single-dose unit preparation can overcome the problem of topical anesthetic abuse.

Thin-Flap Laser In Situ Keratomileusis-Associated Dry Eye: A Comparative Study Between Femtosecond Laser and Mechanical Microkeratome–Assisted Laser In Situ Keratomileusis

Research Abstract

Objective:

To assess and compare postlaser in situ keratomileusis (LASIK) dry eye after LASIK with planned thin flaps created by a femtosecond laser (FS) and mechanical microkeratome (MK).

Methods:

Patients were stratified according to the flap creation technique into FS and MK groups with planned 100 μm flap thickness in all eyes. Dry eye parameters including tear film break-up time (TBUT), Schirmer I test, ocular surface disease index (OSDI), and lower tear meniscus height and area (tear meniscus height [TMH] and tear meniscus area [TMA]) were assessed before surgery and at 3 and 6 months after surgery.

Results:

Conventional mechanical or alcohol-assisted photorefractive keratectomy (PRK) techniques for correction of hyperopia and hyperopic astigmatism were associated with inconsistent results. Transepithelial photorefractive keratectomy: a prospective randomized comparative study between the two-step and the single-step techniques

Research Abstract

Objectives

To assess and compare the six-month outcome of the two-step transepithelial phototherapeutic keratectomy- photorefractive keratectomy (PTK-PRK) and the single-step transepithelial PRK for myopia and myopic astigmatism.

Methods

A prospective randomized study. The study enrolled 100 eyes of 50 patients with mild to moderate myopia or myopic astigmatism stratified into two groups, PTK-PRK (n = 50 eyes) and single step PRK (n = 50 eyes). Primary outcome measures were visual acuity and manifest refraction. Secondary outcome measures were epithelial healing duration, post-PRK pain scores and 3-month postoperative haze grading.

The effect of accelerated pulsed high-fluence corneal cross-linking on corneal endothelium; a prospective specular microscopy study

Background

Corneal collagen cross-linking (CXL) is a procedure utilized for halting keratoconus progression with different approved protocols. The current study aimed to assess the corneal endothelial changes following the relatively new accelerated pulsed high-fluence protocol of epithelium-off corneal cross-linking for the treatment of mild to moderate keratoconus.

احتفلت كلية الطب يوم الاثنين ٣٠ اكتوبر ٢٠٢٣ بتخريج اول دفعة من الدبلومة المهنية في التغذية العلاجية بكلية الطب.

تحت رعاية الدكتور احمد المنشاوي رئيس الجامعة وبحضور الدكتور علاء عطية عميد كلية الطب ورئيس مجلس إدارة المستشفيات الجامعية والدكتورة اماني عمر وكيل الكلية لشئون الدراسات العليا والبحوث والدكتور سعد زكي وكيل الكلية لشئون خدمة المجتمع وتنمية البيئة.

واشراف منسقي الدبلومة الدكتور مدحت العربي استاذ الصحة العامة والدكتور اسامة العشير استاذ طب الاطفال ورئيس وحدة التغذية العلاجية في مستشفى الاطفال ، وبحضور الدكتورة هدى مخلوف مدير وحدة ضمان الجودة ونخبة من أعضاء هيئة التدريس وخريجي الدفعة الاولي والسادة متدربي الدفعة الجديدة .

أكد الدكتور علاء عطية علي سعادته بخروج الفكرة من مجال الحلم الي التطبيق وان المثابرة على النجاح هي طريق الوصول لأكبر الانجازات وهذا ما شهده من كل القائمين علي اعداد وتجهيز الدبلومة ،كما اضاف ان دبلومة للتغذية العلاجية هى من أوائل الدبلومات علي مستوى الجمهورية ومعتمدة من المجلس الأعلى للجامعات ونقابة الاطباء وجامعة اسيوط ويقوم بالتدريس والتقييم فيها نخبة من اساتذة التغذية العلاجية على مستوى الجمهورية.

واشارت الدكتورة امانى عمر إلي اهمية التغذية العلاجية في حل الكثير من المشكلات الصحية وأحيانا تكون الطريقة الوحيدة المتبعة للعلاج في بعض الامراض، او تخفيف حدة الاعراض المرافقة لمرض ما، كما تمنت ان مجهود الدراسة في الدبلومة قد أتت ثمارها مع الأطباء خريجي الدفعة الأولى في اماكن عملهم بالاقسام المختلفة بالمستشفى والتأمين الصحي.

وشكرت كل القائمين علي الدبلومة وكل من شارك في خروجها للنور بهذا المنظر المشرف بداية من وحدة ضمان الجودة برئاسة الدكتوره هدى مخلوف والدكتورة هبه عطية يسي نائب رئيس وحدة ضمان الجوده والدكتورة نهي أبوالفتوح وكل فريق عمل وحدة ضمان الجودة وكل السادة الاداريين بالكلية المشاركين في هذا النجاح.

كما عبر الدكتور سعد زكي عن سعادته بانتمائه الي الدفعة الاولي من الدبلومة الغذائية وكيف كانت تجربة علمية شيقة ممتعه علي مدار عام ومعلومات جديدة في مجال تخصصه الا وهو الجهاز الهضمي وامتاز التدريس بالدقة والنظام والترتيب ، كما دعا كل اعضاء هيئة التدريس لخوض هذه التجربة العلمية لما لها من نفع كبير في العلاج للمرضي وعدم تفردها بتخصص معين.

وقدم الدكتور مدحت العربى نبذة عن الدبلومة المهنية منذ التخطيط لها وحتي الان والمناهج الدراسية بها ،وشكر ادارة الجامعة والكلية على المجهود الرهيب الذى بذل حتى تخرج الدبلومة المهنية فى الصورة المشرفة وقدم الشكر للدكتور يوسف صالح وكيل الكليه السابق لشئون الدراسات العليا والبحوث علي دوره المتميز ودعمه المستمر للدبلومة. واضاف ان الدبلومة للأطباء فقط ،وعدد الدفعة الأولى (٣٠ طبيب) وتجمع خليط من أطباء الجامعة ووزارة الصحة ، ومدتها سنه دراسية (٣٠ أسبوع) يوم نظرى فى الاسبوع ويوم عملي وتم التدريب العملى فى مستشفيات جامعة أسيوط بكل الاقسام الإكلينيكية.

واضاف ا.د/ اسامة العشير ان التغذية العلاجية تعتبر عامل أساسي لصحة جميع الأطفال والكبار، لا تغني عن الدواء وانما هو علاج مصاحب له وان غرض الدبلومة تخريج اخصائي تغذية قادر علي استخدام الغذاء كوسيلة علاجية للاستفادة من العناصر الغذائية الموجودة به.

كما اضاف ان اشتراك الدكتور سعد زكي وكيل الكلية لشئون خدمة المجتمع وتنمية البيئة في الدبلومة مثلا يحتذى به في الشغف لطلب العلم ،ومثلاً في الانضباط والالتزام بالمواعيد بالرغم من كثرة مشاغله.

وفي النهاية عبر الجميع عن سعادتهم بنجاح الدفعة الاولي ووضع اقدامهم ع بداية الطريق الصحيح للتغذية العلاجية وتمنوا للدفعه الثانية النجاح والتوفيق.

قام الأستاذ الدكتور سعد زكي وكيل الكلية لشئون خدمة المجتمع وتنمية البيئة يوم الاحد الموافق ٢٩ اكتوبر ٢٠٢٣ بجولة تفقدية لكافتيريا كلية الطب وكان برفقته السيد محمود حسين امين عام كلية الطب والسيد علاء سرحان مدير الشئون المالية والكيميائية اسماء مسؤول السلامة والصحة المهنية والسيد سعد مدير أمن الكلية والسيد شحاته مسؤول الدفاع المدني والسيد سيد عبد العال مسؤول الصيانة ، وذلك لمتابعة سير العمل داخل الكافتيريا ومدى اتباعها لقواعد السلامة والصحة العامة

في البداية تم مطالعة البطاقات والشهادات الصحية للعاملين داخل الكافتيريا ثم تلاها التأكد من النظافة العامة للمكان وللعاملين والتأكد من اتباعهم قواعد السلامة المهنية من أجل تجنب إصابة العاملين بالكافتيريا او المترددين عليها ، وضمان عدم حدوث اي خسائر جسدية او مادية.

كما تم بشكل عشوائي مراجعة تواريخ صلاحية الأطعمة والأغذية المستخدمة وذلك لضمان الحفاظ على سلامة وصحة أبناءنا الطلاب شباب المستقبل .

كما تم التأكد من تطبيقها لجميع شروط التعاقد مع الجامعة وعدم وجود اى شكاوى من المترددين على الكافتيريا.

تم إطلاق حملة الكشف المبكر عن سرطان الثدي، في منتصف الشهر الجاري؛  وذلك في إطار المشاركة في المبادرة الرئاسية التي أطلقها الرئيس عبد الفتاح السيسي رئيس الجمهورية لدعم صحة المرأة المصرية وتزامناً مع المبادرة العالمية "أكتوبر الوردي".

 تم اثناء اللقاء الختامي عرض تقرير مفصل عن اداء الحملة بالكليات المختلفة ونتائجها.

وقدمت الدكتورة أسماء كمال أستاذ تمريض صحة الأسرة والمجتمع محاضرةً عن: اعراض سرطان الثدي وأهمية الكشف المبكر لسرطان الثدي، والفحص الذاتي ، وآليات الفحص.

كما ألقت الدكتورة هبة بكري مدرس علاج الأورام ومنسق حملة أكتوبر الوردي، محاضرةً عن أهم الحقائق، والخرافات حول سرطان الثدي.

وشهد اللقاء مشاركة ثلاث سيدات تجربتهن مع السرطان ورحلتهن من المرض وحتي تمام الشفاء.

شهد الحفل الوقوف دقيقة حداداً على أرواح شهداء فلسطين، وعلى روح الدكتور مصطفى عبد الونيس رحمه الله، أستاذ الأشعة العلاجية، والطب النووي بمعهد جنوب مصر للأورام، الذي وافته المنية خلال الأيام القليلة .

الجدير بالذكر انه يتوفر بقسم علاج الاورام بالمستشفى الجامعي الرئيسي الكشف يوميا علي السيدات من الساعة ٩ ل ١ ظهرا في عيادة الكشف المبكر لاورام الثدي ضمن المبادرة الرئاسية لصحة المرأة.

انطلاق فعاليات المؤتمر السنوي العشرين لقسم طب الأطفال تحت عنوان " يداً بيد .. من أجل أطفال أصحاء"
